Where To Meet CMO in Pune?

Hi,
I have taken a term life insurance from Exide Life. After the medical tests, they say that I need to get signature of the CMO (Chief Medical Officer) on one of the document they will provide. I don't know where the CMO sits in Pune. If you guys have any idea please let me know. Google searches did not yield good results.
